Filing a Report

Report an incident of relationship violence, gender discrimination, sexual harassment, or sexual violence using the Online Reporting Form.

If you wish to remain anonymous when making an online report, you can omit your name and other identifying information. Please Note: Responsible Employees are prohibited from filing anonymous reports.

Your completed form will automatically be forwarded to the Office of Title IX Compliance.

What Happens After a Report is Made?

Once the Office of Title IX Compliance receives the report, the Title IX Coordinator or designee will:

  • contact the individual(s) identified within the report via email to request a meeting. 
  • assess the situation to determine the safety of the student and others in the campus community. 
  • explain to the person(s) their administrative and legal options, making sure that the student is provided the opportunity to state their preference for moving forward. 
  • At the same time, the University’s Office of Title IX Compliance ensures that the person(s) has access to the resources and support they need both short- and long-term.
