Alternate Operational Information

If the University is operating under Condition 1,  EO may operate under reduced conditions or hours. EO will not be open during Condition 2 operations. For current information regarding University operating conditions, individuals should check their App State email,  Emergency Preparedeness(link is external) and Appstate Alert(link is external) webpages.

On an occasional as-needed basis, such as between semesters or during inclement weather, EO may move operations to remote.

Our office is located at 123 I.G. Greer Hall(link is external) on the side of I.G. Greer that faces Central.  

Requesting Information:

The Office of Access & Equity: Equal Opportunity (EO) adheres to the UNC System General Schedule for the retention of records (GU323); therefore, records are maintained for 5 years after the last semester of enrollment.   EO records are not a part of the academic record maintained within the Registrar's Office.

The release of EO records is governed by the Family Educational Rights and Privacy Act (FERPA).  An explanation of FERPA and policies governing release of records may be accessed at the Office of the Registrar.

Permission to discuss EO/Title IX-related information would be established via the University's FERPA Access option available through AppalNET.

Procedure:

In order to request information be released, submit a DocuSign Authorization Request Form

EO will then send a Docusign Release of Information Authorization to the  email account provided for completion. Once the Release Authorization Form is received and processed, materials will be released via the method requested. Please allow up to two weeks for materials to be gathered and released.

Release methods include:

  • Pick up within EO (photo ID required)

  • Fileshare to an email address verifiable within Banner

  • Mail

  • Fax

Submitting Information:

Materials may be securely submitted via:

  • Fax
  • Hand delivery to the office
  • US Mail
  • fileshare.appstate.edu(link is external) (secure electronic sharing platform). Materials should be sent to the email address: equalopportunity@appstate.edu Documents with more than one page must be combined into one file for submission. PDF format is recommended for files. NOTE: To confirm correct email address you may check Fileshare sent messages. A verification email is automatically sent confirming transmission. For login related issues with Fileshare please contact IT Support Services(link is external).
